Cigna (RN) Cigna Collaborative Care Coordinator (Cheyenne, WY Area) in Cheyenne, Wyoming


The role of Cigna Collaborative Care Coordinator (C4) has been developed as a clinical service model to support Cigna's strategy of collaborating with participating providers to achieve the triple aim of improved quality of health, increased affordability thru decreases in total medical costs and a positive customer and Health Care Professional experience.

The overall goal of the C4 role is to coordinate the delivery of health care services available to Cigna customers through a systematic process of customer identification, case review and collaboration.

Collaboration begins with developing positive working relationships with participating providers. We believe that sharing customer specific actionable information that is timely and appropriate will enhance the current treatment plan and demonstrate value resulting in positive outcomes.

The State of Wyoming (client) requested that Cigna facilitate a collaborative approach with the Wyoming physicians who participate in the State of Wyoming Patient Centered Medical Home and their employees, whereby patients aligned to these physicians will be identified in the suite of actionable reports Cigna produces. The goal is to improve clinical outcomes, reduce inappropriate utilization resulting in an overall cost reduction for both Cigna and the customer.

To support this goal a Cigna employed clinician, (C4 Cigna Collaborative Care Coordinator) will work directly with these physicians to facilitate care management.

The goal of the Cigna Collaborative Care Coordinator (C4) is to work directly with the primary care physician, using actionable information to support care management actions for the State of Wyoming employee population. This initiative supports our goals of leading quality and affordability, winning partnerships with the delivery system and driving localization.

In Wyoming we have aligned all physicians who participate in the State Sponsored Patient Center Medical Home. These physicians will work collaboratively with the C4 and Cigna Care Management Programs and will receive an incentive under certain conditions for doing so. These services include post discharge transition of care visits within 14 days and care coordination/planning with the Cigna case manager.

The ideal candidate must reside in the Cheyenne,WY area. Position is work at home and in the field.


Active RN license required

A minimum of 3 years of direct patient care clinical experience required

CCM certification preferred

Case Management experience preferred

Bachelor Degree in Nursing preferred

Strong computer skills required

Clinical experience in acute care or a clinical setting
